Cardiovascular Medicine, PC will gladly submit claims to all carriers. We participate with many insurance plans, including Medicare, and we will be happy to answer any questions regarding your insurance plan. Plans change frequently; please contact our billing office to verify participation in your plan.

Once a response is received from your insurance carrier, you will be notified of any remaining balance. All co-payments are due at the time of service. For your convenience, we accept Discover, MasterCard, and Visa.

Pre-certification

Most insurance carriers require advance notification for hospital admissions and outpatient testing. While we will assist with obtaining pre-certification, it is the responsibility of the patient to know his/her insurance regulations and to notify his/her insurance carrier per their guidelines. Failure to notify your insurance carrier may result in a reduction or denial of benefits.

Disability Forms

We will gladly assist you in the completion of disability forms as needed. To expedite this service, please complete the patient.s portion, sign the Authorization to Release Information Waiver, and provide us with the appropriate mailing address. Please allow adequate time for us to complete these forms for you.
